undersigned adopter(s) understands that he/she must abide by the
terms of this agreement.
The undersigned
adopter(s) each, separately or jointly, does hereby release and
hold harmless the Montana
German Shorthaired
Pointer Club and
Pointing Dog Rescue,
its members, officers and its rescue volunteers from any claims,
damages or actions incurred as a result of this adoption or
caused by any action of the dog transferred herein.
The
undersigned adopter(s) further agrees to abide by the following
terms:
VEVETERINARY
CARE: I/we will have the dog examined
by a veterinarian.
I/we also agree to
provide quality veterinary care at all times including, but not
limited to, vaccinations and parasite checks. List
the veterinary care and dates care should be completed by below
e.g., rabies update, 7-in-one, worming, etc.

known health conditions and special care instructions
Listed below are all
known behavior issues and special care instructions
Listed below are feeding
instructions, including brand of dog food, amount and number of
times fed

11. TRANSFER
OF OWNERSHIP: I/we will never take or allow this dog
to be taken to an animal shelter, pound, humane society or
other
rescue, nor will I/we transfer ownership of this dog to any
other person, firm, corporation or organization without prior
notification and approval of Nancy
Lane . If for any
reason, at any time, I/we need to find another home for this
dog, we understand and agree that it MUST be returned to Nancy
Lane . Sign
12. CARE:
I /we will always treat this dog
as a house dog and
family pet. I / we will provide secure housing (fenced yard,
crate, dog house etc.)
as needed.
I/we
agree to keep this dog on a leash or a confined area for at
least 10 days or until the dog is reliably responding to the
"come" or "here" command.
I/we
agree to exercise the dog regularly in a properly supervised
environment. I/we understand that th is
is a dog that has high-energy needs and will provide exercise on
a regular basis. I/we will treat the dog with affection and
kindness and never subject it to any type of abuse or neglect.
I/we will care for the dog humanely by providing the food,
water, shelter and veterinary care necessary to keep this dog in
optimum health. I/we will never let this dog be used for any
medical or experimental purposes, nor for attack or guard dog
purposes.

13.
VISITATION:
I/we understand that Pointing Dog Rescue may make periodic house
visits and/or telephone inquiries to insure the
well-being
of the dog that I/we are
adopting.
I/we agree to
cooperate fully and truthfully with Pointing
Dog Rescue's
inquiries and to allow Pointing
Dog Rescue to see the
dog
upon request.
I/we understand that Pointing
Dog Rescue has the
right to take possession of the dog, at its discretion, if
during a visit Pointing
Dog Rescue determines
the dog is not being properly cared for. Sign
14.
ADOPTION
FEE: I/we agree to the adoption fee listed below upon receipt of
the dog. (There will be a minimum $50.00 handling charge
for
all returned checks.)
15. IDENTIFICATION:
I/we agree to keep
proper identification (Personal ID and License tags) on the dog
at all times.
Pointing Dog Rescue
recommends tattooing or microchipping with a registered agency.
If this dog already has such identification, list

16. LOSS:
If this dog
is lost or stolen I/we will immediately notify Nancy
Lane and make every
effort to locate the dog by
placing
newspaper ads and calling local animal shelters and police
stations in the area. Sign
17.
RETURNS
AND REFUNDS: If, for any reason during the entire life span of
this dog, I/we are unable to keep this dog or if
I/we
fail to comply with the terms of this contract, I/we agree to
return the dog to
Nancy
Lane . If, within 10
days,
I/we
the
dog
unsuitable and return it to Nancy
Lane in good or
better mental and physical condition than it was on the date of
the adoption,
all
of my/our adoption fee will be refunded.
I/we realize there
will be no refund after 10 days.

18. SPAYING/NEUTERING:
If, for reasons of age or health, this dog is not yet spayed or
neutered, I/we agree to have this dog
spayed/
neutered by
I/we understand that under no circumstances may this dog be used
for breeding. I/we agree to send
Nancy
Lane
a copy of the spay/neuter certificate signed by the veterinarian
who performed the surgery. I/we further agree not to use
this
dog (whether sire or dam) for breeding purposes. If the dog is
used for breeding prior to sterilization, I/ we agree to pay the
sum
of
one
thousand
five hundred ($1, 5 00.00)
for liquidated damages to Pointing
Dog Rescue , and that
resulting offspring, as well as the adopted dog, become the
property
of Pointing
Dog Rescue. Sign
19. ATTORNEY
FEES AND COSTS: I/we agree to be responsible for any and all
legal fees incurred if a violation of this contract
results
in
a lawsuit. or any litigation.
I/we
understand that this contract is binding in the United States of
America and that noncompliance with any of the terms of this
agreement,
including failure to pay or problems encountered in the
collection of the adoption fee, may result in legal action and
seizure
of the dog.
